Before participating in any diver training, a new students must complete a BSAC Self-Declaration Medical Form (to be renewed yearly). This completed the form should be sent to the BSAC Administrator. You need to be able to answer “NO” to all questions before commencing training.
Should you however answer "YES" to any question, it may still be possible to undertake the dive training provided you have a medical examination and be certified fit to dive, by a doctor.

Most Dutch "huisarts" are willing to carryout this medical for you & complete the form. Once completed & signed, the fitness to Dive Certificate (which forms part of this medical form[1]) should be shown to the BSAC Administrator. In the event that your huisarts is unwilling to do this, you can arrange a medical at the:

[1]The BSAC Administrator does not need to see the full medical report; he/she needs only to confirm that the Fitness to Dive Certificate has been signed & stamped.
