application.
2) Print a copy of this form. If you are
unable to print now, please save this page as a HTML document in
your own device (e.g. diskette/zipdisk/thumbdrive) for your own
printing later. |
3) BY COLLECTION |
If you have opted to collect your
transcript at the Student Service Centre (SSC) counter of the Yusof
Ishak House (Level 1), No. 31 Lower Kent Ridge Road or at Bukit
Timah Campus Management Office (BMO), Block B, 469 Bukit Timah Road,
please do the following: |
(a) |
Bring this form and make payment (NETs/Cheque) at the
respective collection points. Please note that the Cashier is open
from 8.30 am to 5.30 pm (Monday – Friday). |
(b) |
Produce your NRIC/Passport for verification purposes. |
(c) |
If you are sending a proxy to collect your
transcript, he/she must produce an authorization letter duly
completed and signed by you, your payment receipt, a copy of your
NRIC (both sides) or Passport (the personal particulars page), and
the proxy’s NRIC/Passport. |
4) BY POST |
If you have opted for your
transcript to be sent by post, please do the following before we can
process your request: |
(a) |
Full payment to be made |

• By cheque payment:
Send this form and your cheque made payable to “National University
of Singapore” (indicate on the back of the cheque: name, degree and
“Transcript Payment”) to Registrar’s Office , NUS, University Hall
(Lee Kong Chian Wing), #UHL 04-01, Lower Kent Ridge Road, S(119077). |

• Payment at NUS Cashier:
Bring this form and make payment at the NUS Cashier. Once the
payment form is machine receipted, it will be forwarded to
Registrar's Office as the official receipt to confirm payment of
your transcript(s) application. |
(b) |
Submit a photocopy of your NRIC (both sides) /
Passport (the page with the personal particulars) to the relevant
office processing the transcript if the transcript(s) is/are to be
sent to a stated mailing address. |
