Before opening a live trading account, please be sure to familiarize yourself with our Trading Terms.

Notice: Before you can begin trading, you must complete the following procedure:

Select an appropriate agreement from the table below.
Fill out and sign the applicable pages.
Fax/Mail/Email the above pages, and copies of two valid identification documents to us.
Immediately notify us that you have sent the documents by emailing account@cms-forex.com.
Funds should be sent ONLY AFTER APPROVAL of your account.

IMPORTANT: Only the following types of deposits are acceptable:

Personal Check (USD deposits ONLY)
Wire Transfer
Inter-Account Transfer
PayPal (USD deposits ONLY)

*Please note: CMS clients may incur additional costs assessed by their Bank or other intermediary financial institutions. Capital Market Services, LLC will not
be held responsible for any additional charges or fees for transferring funds via wire or check from your designated institution. Only the net amount received
by CMS will be applied towards the trader's account balance.

All other types of payment will be returned at the owner's expense!

IB-Referred
Individual

NOTICE: Joint accounts between an Introducing Broker and an individual are not permitted.
Fill out secure online contract, then print, sign, and fax/mail/email the last page.
The "Disclosure for Clients Introduced to CMS by Third Parties" page must also be filled out, with the IB's
name included where applicable.

If you experience problems registering via the online agreement, you may download the agreement, fill
it out, sign, and fax/mail/email pages 1-6, 13, 14, 16-18.

Joint-Account Holder

NOTICE: Joint accounts between an Introducing Broker and an individual are not permitted.
Fill out online contract, include IB's (Introducing Broker) name where applicable, then print, sign, and
fax/mail/email the last page. The "Disclosure for Clients Introduced to CMS by Third Parties" page must also be
filled out, with the IB's name included where applicable. This page can be downloaded by clicking the PDF/DOC
download links to the left.

If you experience problems registering via the online agreement, you may download the agreement, fill it out, sign,
and fax/mail/email pages 1-6, 13, 14, 16-18.

Corporate Client

NOTICE: Joint accounts between two corporations are not permitted.

TRUST

NOTICE: Joint accounts between two trusts are not permitted.

If you authorize a third party to trade on your behalf, then fill out page 20.
If you were introduced by a third party, then fill out page 21.
In addition to the completed application, please submit the following:
Trust Agreement;
Documents confirming formation of Trust; Copies of two forms of legal
identification, including photo identification, of all individuals authorized
to trade on Trust’s behalf; Bank reference (if available).

Transfers

Deposits are now available not only in USD, but also in 7 other world currencies - JPY, EUR, AUD, CAD, CHF, GBP
and NZD, without their conversion.

Wire transfers are cleared within 1-2 business days after receipt.

NOTICE: Funds wired before the trading agreement is approved will be immediately returned at the customer's
expense!

Inter-Account Transfer:

Download and complete the Account Transfer Form: NOTICE: We may transfer money from one account to another if both of them belong to the same trader only. No funds transfers from one account to another made via telephone requests are accepted!

PayPal (USD deposits ONLY)*:

You can wire funds for trading via PayPal. The CMS PayPal account is: pay2@cms-forex.com. Visit PayPal.com for
more information about this service. PayPal transactions are usually completed within 3-4 business days after
submission.

*Please note: CMS clients will incur additional costs assessed by PayPal, a company of Ebay. Capital Market
Services, LLC will not be held responsible for any additional charges or fees for transferring funds via PayPal,
whether charged directly to the client or to CMS.

Withdrawing Funds:

We will process your request within 2-3 business days of receiving your request. Be sure to include the correct wiring instructions, ABA or SWIFT number,
bank name, address, and account number.

Transferring Funds Between your CMS Account and an Account at Another Brokerage:

To request a transfer of funds from your CMS account to another brokerage, please open the Withdrawal Request form by clicking on the link above.

In the section entitled "Your Bank Information," please indicate the full bank information for the brokerage that will be receiving the funds. You should indicate
the following as the Beneficiary Name: "(Brokerage Name) - For Further Credit to (Your Name)."

To request a transfer of funds from another brokerage to your CMS account, please file a request for withdrawal of funds with that brokerage. In the "Bank
Information" section on their request form, please indicate CMS' full bank information. You should indicate the following as the Beneficiary Name: "Capital
Market Services, LLC - For Further Credit to (Your Name)."

What to Send Us:
Fax/Mail/Email copies of two government issued photo identification documents (ie: passport and/or driver's license),
and the pages specified in the below table, under the type of trader which applies to you.

If emailing account opening documents:
In order to avoid unnecessary delays, please be sure to include the word "APPLICATION" in the subject line.

Important Information About Procedures For Opening a New Account:
To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial
institutions to obtain, verify, and record information that identifies each person who opens an account. What this means
for you: When you open an account, CMS will ask for your name, address, date of birth and other information that will
allow CMS to identify you. CMS may also ask to see your driver’s license or other identifying documents.

problems, download, print, and complete the entire agreement, including
the Managed Account Authorization portion. Fax, mail, or email all of
these documents, including your photocopies of government IDs, to CMS
at the address below.
