A Loving Connection between Children with Disabilities and Resources They Need

Temporary Financial Assistance Available

To be Eligible for this program, you muse meet the following criteria

• Have a child with a physical or mental disability diagnosed within the last 6 months (the diganosis can be prenatal or at any age)
• Be at or below the State of New Mexico's poverty line

To apply, please do the following:

1. Pick up an application from ChildLink (address below)
2. Assemble the following papers:
   1. Your completed application
   2. Doctor's record of yoru child's diagnosis
   3. Tax records or New Mexico Social Services certificate of yoru income level
   4. Your lease, mortgage, or medical bills, depending on which you need help with
3. Make an appointment with ChildLink volunteer, available these times:
   1. Ida: MW 10:30 a.m. to 3:30 p.m.
   2. Juan TR 9:00 a.m. to noon
   3. Chris: F 10:30 a.m. to 3:30 p.m.
