The Gathering 2000: Orlando
Parental Consent Form
The Gathering 2000 - Orlando, Florida is a convention celebrating Gargoyles, Disney's animated television program. Events will include panel discussions with special guests, a masquerade contest, an art show, various games and activities, and catered dinner.
Minors between 12 and 18 attending unsupervised must return this parental consent form, signed by a parent or legal guardian, along with their registration form and fee, if they wish to participate. Any minor who does not have a parental consent form will not be permitted to attend the convention, and their registration will be returned unprocessed.
If your child is planning to attend the convention alone, travel and rooming arrangements are the responsibility of the parent or legal guardian. G2K will NOT be assigning rooms or be acting as an intermediary with the hotel. However, a comment room will be available soon for attendees to use to assist them in finding roommates.

Convention Information:
I agree not to hold The Gathering 2000 (G2K) or any of its members liable for any expenses or injuries that my child may incur or cause while attending The Gathering 2000. I understand that my child is responsible for his/her own behavior at all times. If, in the view of the senior convention staff, my child becomes involved in behavior that presents a danger to him/herself and others, appropriate steps will be taken to protect all participants of the convention.
I agree that, in the event of an injury, the members of the senior convention staff may act on my behalf in obtaining medical treatment for my child. I have indicated on the back of this form any permanent or temporary conditions that should be known about my child.
