Master Firoz is suffering from severe Hemophilia A, which is a serious congenital (inherited) serious disorder of the coagulation of blood. These patients have external or internal bleedings following minor or trivial injuries or even spontaneously. This is due to an inherited (genetically determined) deficiency of factor VIII, which is an important factor responsible for stopping bleeding in normal individuals. Management of Hemophilia A needs to be very comprehensive and has to be done by a team involving pediatric hematologist, pediatric orthopedic surgeon, physiotherapist and occupational therapist, dentist, genetic counselor etc. But the mainstay of treatment is prompt replacement therapy by commercially available factor VIII concentrates to control or prevent bleeding, and to prevent long term complications resulting from internal joint and muscle bleeds. Master Firoz is 7 years old and hails from the interiors of a developing country like India, where none of the abovementioned facilities are available. He suffers from severe Hemophilia A and is already a cripple due to recurrent bleeds in his muscles and joints leading to chronic arthropathy (Painful swellings and stiffness of joints and wasting of muscles). If not attended he may be left a total cripple due to irreversible damage, ultimately with fixed unusable joints. Master Firoz, if he is to be helped at this stage, would require continuous supply of factor VIII- both to prevent further bleeds as well as for his rehabilitation. His rehabilitation program would include intense physiotherapy, intra articular injections (injections in the joints) and may be surgery-all under cover of factor VIII. He would need about 500 units of factor VIII almost everyday or every other day for an indefinite period (1-2 years) and less frequently thereafter. Factor VIII is not available very easily in India and when available costs 10 to 16 Rs/- per unit (comes to about Rs. 5000 to 8000 per dose for Firoz).
