The Michigan Soccer Association Registrar has given me guidelines that I must follow THEREFORE; Team Managers must also comply with the following
MSA TEAM
AFFILIATION/REGISTRATION ROSTER
-The player
ID # is as follows (league # (004), team #, and # stamped on the back of the ID
Card. -Do not cut and paste -Do not skip lines.
-DO NOT CROSS OUT
ANY PLAYERS ON THE TEAM ROSTER! ! ! ! (YOU WILL PAY FOR ANY PLAYERS CROSSED
OFF!)
-Have players sign
the Waiver Form on the # their name appears on the front of the roster
-The entire
ID card should be filled out front and back!!! I should only have to
fill in the date on the back and sign the back on the line Registrar.
-REMEMBER! ! ! ! !
The S.S. # is not used anymore!!! The ID # is now the league # (004), team #
and # stamped on the back of the ID card.
-ATTACH THE PHOTO
USING GLUE STICK OR TAPE (DO NOT STAPLE)!!!!!
-The team managers
are accountable for the blank ID cards and your team WILL BE CHARGED IF THEY
ARE LOST, DAMAGED, etc. If you make a mistake, use white out
-NEW! ! ! ! The S.S.
# is not used on the A form. Use the player ID#.
-The player section
(top portion) and team representative section (middle portion) need to be
filled out completely.
-BOTH THE PLAYER AND
THE TEAM REPRESENTATIVE MUST SIGN AT THE BOTTOM OF THE FORM.
-I SHOULD ONLY HAVE
TO SIGN AT THE BOTTOM OF THE FORM!!!!!!!!!!!!
-EVERY NEW
PLAYER MUST INCLUDE A COPY OF THEIR DRIVER'S LICENSE AT THE TIME OF
REGISTRATION
-If you add a player
who previously played with another team a NEW COPY OF THEIR DRIVER'S LICENSE IS
REQUIRED.
