GENERAL INFORMATION ABOUT FILING A CLAIM

1. If a player is injured at a practice or in a game, she is covered. Players are not covered while they are driving to or from a game or practice.

2. In order for the player to file a claim you MUST:

1. Make sure that the injury is noted on a game report by the referee.
2. Fill out the proper form (Accident Claim Form)
3. Submit the proper documentation within 90 days of injury.

3. Be aware that the claim form must be signed and dated by a League officer.

4. You must include a copy of the GAME REPORT.

5. There is a $400.00 deductible, and the policy coordinates benefits with any other insurance coverage the player might have.
