I have a Masters in Clinical Social Work and my role is to work with all the regional coalitions/groups around chronic disease prevention programs by providing technical assistance and support to help sustain the groups that are active. The state has state plans for each program and within my role I attend all of the statewide meetings and bring back to the regional groups/coalitions this information as well as information about what is going on in the Southeast Regional communities as well as other regional communities in regards to chronic disease prevention programs. What is working/not working, what the communities are working on based on what they have identified as a problem and targeted group. The state DPH has many programs that it funds and works with and although funds have been cut tremendously in some areas, the state has many representatives that are actively working with communities.

High Point Treatment Center's Transitional Support Services ("T.S.S.") Program, referrals from Department of Public Health funded Detoxification Programs and Shelters in Southeastern Massachusetts. A safe, supportive, substance free and therapeutic environment to enhance and promote the recovery process and assist residents to maintain abstinence, bridging the gap between detoxification and aftercare. Service elements include case management and counseling (ie., service planning, goal development, family unification), Psycho-educational groups and lectures (relapse prevention, relationships, HIV awareness, domestic violence, nutrition, self-help, Men's and Women's recovery issues) daily in-house commitments (AA / NA) groups and recreational activities. Referrals for those residents requiring physical and mental health care, assistance with legal issues, sober housing, residential treatment, and vocational and educational needs will be facilitated. Transportation provided to appointments in the community. All residents have access to area doctors, dentists, mental health specialists, vocational and educational programs. Interviews to after-care facilities (ie., sober housing and other treatment modalities) also arranged.
