The Integra Type-R manifold onto the GSR head can be very tricky.  For ease of installation I recommend having a GSR intake manifold gasket and possibly an extra GSR head to line the manifold up with.
In order to have the holes line up correctly use the gasket to line up the holes and a sharpie to draw them in.  I recommend a drill press to drill the holes, but a hand held drill should suffice.  You can go a little bigger than stock and still be fine with the bolts.
Notice I used putty to weld on the pipe where the old one used to be.  Also the R manifold has been cut to allow the welding of the pipe as the coolant locations are different.  You also need to have a machine shop shave the top center of the manifold so you can use the center nut 12mm(Sorry no pick). 
I am using the Integra Type-R throttle cable, and bracket.  You can continue to use the GSR one if you prefer, but the strut bar will not fit because of the throttle cable bracket.
This conversion is quite labor intensive.  I did however gain over 7hp and 4lbs of torque with the conversion.  This was done before the Skunk2 manifold came out.  I heard it makes the same power.  It was worth the effort in my book.
