U Weleerwaarde verzocht om het Heiligen Doop te willen bedienen aan het kind die genaamd word Gasina Maria Wilhelmina, geboren den 8 Juni 1821, waarvan de ouders is Pertus Johannis de Wit en Jacobina Elisabeth de Wit geboren Koek en de Doopmoeder en Doopvader Mevrouw Gasina [...] Thijssen geboren [...] en den Heer [A..?] Koek.

Reverend Sir,

[You are] requested to administer the Holy baptism to the child named Gasina Maria Wilhelmina, born on 8th June 1821. The parents are Petrus Johannis de Wit and Jocobina Elisabeth de Wit, nee Koek; the baptism mother and the baptism father are Mrs. Gasina [...] Thijssen, nee [...] and Mr. [A..?] Koek.

Note :
1. Gasina Maria Wilhelmina de Wit's godfather is noted by the translator as Mr. [A..?] Koek. We suspect that this is in fact her grandfather, Mr. Adriaan Koek.
