Child Care Agreement Between Provider and Parent

Business Hours: The daycare is open from 7:15 a.m. to 5:15 p.m., Monday through Friday.
Pick Up: You agree to pick up your child(ren) on time. An additional fee of $5 per half hour will be charged if you are late. In the event of an emergency, you agree to notify me as soon as possible and let me know what time you will arrive. You will sign a form prior to your child's first day of care indicating any individuals who are authorized to pick up your child(ren). You will agree to notify me prior to any day one of these individuals (other than yourself) will pick up the child.
Fees: The fee for care will be $ per week, if full time. If part-time, the fee per hour will be $ for at least hours per week. There will be a $10 fee for any returned check. After two (2) returned checks, I will accept cash only.
In order to hold your child(ren)'s enrollment in the daycare, this fee must be paid whether your child is here or not.
This fee includes morning snack, lunch, and afternoon snack; but does not include diapers, wipes, formula, or anything else not listed above.
You agree to pay this fee in full every Monday morning, which will be care for that week.
This contract will be reviewed in three (3) months and at that time, the fee and any changes will be addressed. Also, if either one of us needs to back out of this contract, there will be at least a two week notice. Personal/Sick/Vacation Time: Please see the attached sheet of the nine (9) holidays that I will be paid for. I will take one full week in the summer (unpaid); one week between Christmas and New Year (unpaid); five (5) paid vacation days; and two unpaid vacation days (the Friday before Memorial Day and the Friday before Labor Day). I will be paid for two (2) personal/sick days a year.
Sick Time: In the interest of all children, it is best to keep a sick child at home if they have any of the following conditions: a contagious disease, a fever of 101 or higher, or any other illness that requires an excessive amount of individual adult attention. As a parent, you know best when it is more appropriate for your child to be comforted by you at home than being sent to a daycare home. As a courtesy, when your child is sick and not coming, please notify me as soon as possible (even the night before would be appreciated). You will still be responsible for payment for that day.
If your child becomes ill in my home, I will notify you as soon as possible to discuss treatment and/or early or immediate pick up. Your child must be picked up within an hour whenever possible, unless a mutually agreed upon time is set. You agree to accept and trust my judgment of how sick your child has become while in my care.
If your child begins taking an antibiotic, he/she must stay home for at least 24 hours after the medication has been started. If I need to administer medicine, you will need to sign a medical form giving me permission. I will also not administer medicine to your child if they have never had this medicine before. I do not want to be the first one to administer medicine in case of a reaction.
Other: As a courtesy, when your child is not coming (for any reason), please notify me as soon as possible (even the night before would be appreciated). You are responsible for selecting back up childcare arrangements when I am unavailable. I agree to offer an atmosphere of warmth, caring, and concern for all the children in my care.
I will offer nutritious morning and afternoon snacks and lunch, and provide beverages for every child. I will take responsibility for strollers, clothing, or other items belonging to the child. You will agree to provide me with all the necessary forms and telephone numbers where you or a responsible relative can be reached at all times on or before the first day your child(ren) is in my care.
You will give permission for your child(ren) to ride in my car; I agree to use an approved car seat, booster seat, or seat belt as appropriate. I will make every effort to plan trips in advance and to notify you of such trips. You will agree to let me transport your child(ren) without notice in an emergency situation (i.e., my child becomes sick at school and needs immediate pick up). I will inform you if such a situation has occurred. This agreement is effective as of .
