  4. If married within the year, state month. (Jan., Feb., Mar.)
  5. Attended school within the year
  6. Cannot read.
  7. Cannot write.
  8. Whether deaf and dumb, blind, insane or idiotic.
  9. Male citizen of U.S. of 21 years of age and upwards
  10. Male citizen of U.S. of 21 years of age and upwards, whose right to vote is denied or abridged on other grounds than rebellion or other crime.
