EVERYONE SAY "CHEESE"

1. Please read carefully, and mark your calendar.

2. Choose a background and a package from the selections below. If you do not choose a background, the traditional Blue Background shall be used.

3. Please make your CHECK payable to LIFETOUCH.

4. For each child, please use a separate envelop with a separate check.

5. The 5th Grade playground picture will be taken on that day, weather permitting.

6. All gym requirements are suspended on Picture Day.

7. Please remember to return your order forms and checks no later than Friday October 4th.
