This form is to be used if a member school’s student is participating in an event that is not sanctioned by the B.C.S.S.M.A.

This form was created by the B.C.S.S.M.A to provide sponsor teachers of member schools with a format for notification and approval by parents for students wishing to participate in events that are not sanctioned by the B.C.S.S.M.A. Parental forms that have been completed for the sanctioned events are not to be used at any other events. New Parental Consent forms for non-sanctioned events must be completed for each of the events.
