Notes on Filling Out This Form:
- School year - in the form of "2000-2001"
- Parent's name and address - Phone number is optional according to the regulations, but school-supplied forms don't always tell you this.
- Other teacher's name and address - This one will be blank for most people. Folks like piano teachers need not be listed. Like #2, phone number is optional.
- Full name and Birthdate - You can use one form for all of your children; you don't need to fill out a separate form for each child. You do not need to supply any information like shot records, social security numbers, etc.
- Inclusion - Just put a check on the little line. The eight categories are listed, but the specific topics within those categories are completely up to you.
- Outline of intended curriculum - and
- List of textbooks, etc - These two items usually seem the most daunting, but they do not need to be very detailed. Notice that they are only for informational purposes. This means two things:
- Your excuse cannot be denied based on what you do/don't list here, and
- These cannot be used as criteria to gauge whether or not your child is being "properly educated" when it comes time for assessment.

- Assurance of 900 hours - Again, this is just a checkmark. This does not mean you have to schedule (or even document) 180 five-hour school days! Most home educators find it very easy to get 900 hours of education in well before the end of a public-school year.
- Assurance of qualification - Just check the line by the number. You do not need to indicate which qualification you have. Nor can you be compelled to provide "proof" that you have one of the listed qualifications. This is explicitly stated in a letter from the Ohio State Board of Education to all Ohio superintendents (Question #9) in 1993. Many school-suppied forms ask you which qualification you have, but you do not have to tell them this.
- Signature - One signature affirms all the statements made on the notification form. Don't forget to sign and date the form!
