Originally Victor Salva came up with the "every 23 years" credo for the Creeper to guarantee there would be no sequel.  After the relative success of the first movie, though, you had to know the second would be made.  And it happens during the same 23 day hunting spree that occurred in the original (we were never told which day of the span during which those events took place!).  On the 22nd day, the Creeper poses as a scarecrow, and flies away with a young boy.  The boy's father is very distraught, and vows revenge.
 
On the 23rd day, the Creeper flattens two tires on a school bus that is carrying the state champion basketball team back home from their victory.  He then systematically attacks the stranded coaches, players and cheerleaders. 
 
Not much of a story, but some good tension with everyone being stuck on a bus.  The acting is better than you would think coming from a high school basketball team, but the story hardly fleshes out any of the characters.  The creeper himself is ok.  Not a very charismatic killer, and not really all that scary.  But he's got an interesting look (I like him better in the trenchcoat & hat than with his bare skin & wings).  Anyway, there's no real connection from the first except the creeper and a cameo.  There is some gore, but hardly any blood.  An ok horror flick, but a disappointment after the (somewhat) originality of the first.
