Last week, the FDA gave Vivus, Inc. clearance to sell its MUSE
(alprostadil) urethral suppository for the treatment of erectile dysfunction.

MUSE provides a dose of a type of a synthetic prostaglandin called
alprostadil. It does not require any injections or pumps, and consists of a
very small suppository that is inserted about 1 inch into the urethra with a
disposable plastic applicator.

A study of 996 men with organic erectile dysfunction in a 3 month
home-treatment study found that about 65% of the men using MUSE had
intercourse at least once, compared to 19% of the men using a placebo.
Among the men for whom MUSE was successful, using MUSE resulted in sexual
intercourse about 7 times out of ten. The most common side effect reported,
mild and transient penile pain, occurred in about 11% of use.

The company expects to begin shipping the product in early 1997.

Paula,

Thanks for the post about MUSE. I have been hearing about this product
for a long time, and have been waiting for a concrete announcement.

I was one of the first people to try papaverine injections several years
ago, but while they worked wonderfully, they were extremely painful for me.
I'm very hopeful this new process works as well, without the painful
injection.

I called the MUSE Info line and there is a packet of info that should be on
the way here now. The gentleman who answered told me that I was one of the
first callers who was not a medical professional but that he expected many
more calls because there had been a newscast
about MUSE the night before in San Antonio.

The info he gave me over the phone was.......it should be in the Pharmacies
by the second or third week in January......that

Urologists......that they would not recommend it's use more than twice a day
(I must admit I did giggle at this one..wanting to comment about making up
for lost time but I didn't), that erections last anywhere from 30 to 90
minutes and that it would come with the recomm recommendation that it be used
in the doctors office the first time.

As soon as the info packet gets here I will post more info.

significant unanswered question now is about the cost. Just how much is an
old run up the flagpole going to cost, anyway?

BTW, I find it extremely humorous that the two people discussing it here
(you & Paula) are *female*! Hmmmm...

Somewhere around mid January I'll let you know the *male* perspective on
how it works. I plan to be standing in line when it shows up.

<s> One way or another we have to get the word out Larry :) It isn't exactly
the easiest thing to discuss in public....but it is important and Linda
taught me a few years ago that if I really wanted to be able to help SO's
here on the forum I needed to be able to discuss impotency.

The approval of MUSE by the FDA sounds like it will be a wonderful step
forward for diabetic guys and their SO's too. <blush>

Because of the long term effects of injectibles....Neil's Urologist refuses
to recommend them. Hopefully there are no side effects that will have to be
worried about with MUSE either.

It's interesting that it will only be available through urologists.
Perhaps that's a wise idea. It also seems wise to do the first application
in the office, to get the knack of applying it without unnecessary pain --
here's hoping that will be done with a free sample! (g) It sounds terrific
that they will be able to start distributing it in January --what a promising
way to start the year!

controlled by the pharmacies I wonder? If they only ship it to Urologists,

Cat it will be expensive, I figure around 20 dollars a shot <G> But in some
cases insurance will pay up to 80% of that . If you want I can give you a
contact in a P message who can explain this to you. Caverject is available in
any pharmacy with a RX from an MD .

Neil's Urologist will not prescribe injectibles so Caverject or any of other
injectibles made up by a Urologist are not, for all intents and purposes
available to us. Yes, he could change Urologists, but he likes the one he
has and trusts his judgment that the long term effects are not worth it.

From the information I had found and posted here a year or so ago...and from
what I was told on the phone on the MUSE Info line..... MUSE is a small pill
that is inserted in to the end of the penis with an applicator of some sort
which begins to dissolve when a small amount of urine passes through the
penis.

As soon as we get the information package from them, I'll get the info posted
here.

They showed the MUSE applicator on Network News last night. The "pill" is
tiny and the applicator tiny. According to CBS (I think) it is going to be a
huge money maker as the baby boomers join the 11mm diagnosed impotents
already in the market. I expect they feel $25/use is cheap. I can't imagine
most folks using it more than 1x per day, but that would be AT LEAST a $30mm
a month or $3.6 BILLION a year industry of it's own to start.

of Impotence -Sixty-five Percent of Women are Unaware of Treatment
OptionsAccording to a national survey conducted under the auspices of
theImpotence Institute of America (IIA), a majority of adult American
womenare happy with their sex lives and expect to be sexually active,
onaverage, to age 75. Yet many of these women may find their
sexualexpectations unmet, in part because of male impotence which affects
abouthalf of all men by age 70.While impotence can thwart sexual expectations
of the woman, the surveyfound 80 percent of women surveyed whose partner has
experienced impotencehad not encouraged their partner to seek help. They
didn't believe thattheir partner could be helped by treatment. In fact, 65
percent of thewomen surveyed were unaware of any available treatment options
forimpotence.The IIA survey, released in connection with Impotence Awareness
Month(September), was sponsored by an educational grant from The Upjohn
Company.The phone survey explored the sexual behavior, knowledge and
attitudes ofadult women (18-50+) toward male impotence and its potential
effect ontheir sexual relationships.Sexual Longevity and ImpotenceEighty
percent of the women surveyed consider sex to be an important partof their

Great article Joe. And some of the very things that are mentioned in it are
why I do try to post or follow up on messages about new impotency treatments.
I also am not hesitant to mention impotency in conferences when people ask
about complications. As you know it is not something that is easily
discussed by many people, men and women alike. Just saying that it is a
common problem can lift a lot of weight from people's shoulders even if they
can't bring themselves to talk about it online.

I try and find a happy medium in messages.....get the word out, but not be so
graphic that people will call CSI and complain.

Hi Paula, I am glad to see that this topic is being addressed outside of the
confines of the "private sections".
At this point alprostadil is available and BTW is very effective in an
injectable form. The commercial name is Caverject and has been approved by
the FDA since last year. It is a kit with a vial of powder and a liquid
diluent. These have to be mixed and injected into the Cavernous tissue of the
male organ. The syringe comes with the kit. This injection dilates this
tissue and permits blood to flow in producing an erection which can last up
to 2 hours. Since impotence is a very common complication of DM this
substance has been a godsend to millions of DM's and non DM's who have
erectile dysfunction and their mates as well. Impotence is a problem that
affects not only the impotent but his mate as well.

There are other injectables that will produce the same results, papaverine
and prostaglandine. Which are not FDA approved but are Rx'ed by many
urologists because they are just as effective as Caverject and much cheaper.
A Caverject kit (comes in 2 dosages (10 and 20) costs about 20 dollars, a
vial of papaverine good for about 10 injections costs about 40 dollars. And
may be injected with a 30 gauge insulin syringe which BTW is much better than
the syringe that comes with the kit.

This is not a "cure" for impotence it is a "treatment" much like insulin is
for DM. And a major drawback is that after a few years of injections you may
have complications like scarring and fibrosis. Alhough in MHO the rewards of
renewing an active and very good sex life are worth the risk. And you can
always opt for the second stage scenario which is a penile implant.

MUSE is an acronym for Medicated Urethral System for Erection and if the hype
is true, it will truly be a blessing for the 10 million or so impotent men in
the US and millions in the rest of the world. The promise is that it will
work just as well as the injectables without the discomfort of an injection
and reducing the risk of fibrosis significantly.

A new treatment for impotence has been successful in more than 70
percent
of men tested -- and does not require shots, surgery or pumps.

Instead, it uses a device called the Medicated Urethral System for
Erection (MUSE) -- a small plunger, about one-eighth-inch diameter at
the
tip, that
is briefly inserted into the urethral opening, where it releases a
pellet
of erection-inducing medication that is absorbed into the mucous
membranes.

Typically men with erectile dysfunction self-inject the penis with
muscle-relaxing drugs, which dilate the blood vessels. That method,
however, sometimes results in a prolonged and painful erection,
impotency experts say.

"The new system is a real breakthrough," said Dr. Culley Carson III, a
urologist at the University of North Carolina School of Medicine in
Chapel Hill. "It's going to improve the ability of men to use medication
to
restore their erections in a way that we haven't been able to accomplish
before."

In most cases, erectile dysfunction is thought to be caused by physical
problems like diabetes, excess alcohol consumption, smoking, multiple
sclerosis or the side effects from prescription drugs.

In a study of 234 men who had not achieved an erection for an average of
four years, 76 percent of the men achieved an erection after the MUSE
treatment. Less than 3 percent of men who were treated with a placebo
experienced the same effect.

Researchers say injecting erection-producing drugs is generally more
effective, but that the MUSE treatment will probably prove more
attractive
to many men.

"Given the choice, most people would use a little pellet at the end of
the urethra rather than injecting medicine," said Robert Krane of the
University of Southern California School of Medicine.

The device, manufactured by Vivus Inc. in Menlo Park, Calif., is
expected
to be on the market by 1997.

early stages of testing, significantly improved erections and
sexual performance in the majority of men who took the drug,
according to a British study presented here.

Though preliminary, the research is important because there are
currently no effective oral agents to treat the condition, said a
New York researcher.

The drug, sildenafil, produces more natural erections by
enhancing the erectile activity that occurs with sexual
stimulation, said lead researcher Clive J.C. Gingell, M.D., a
consultant urologist at the Bristol Urological Institute in
Bristol, England.

Specifically, sildenafil inhibits phosphodiesterase-5, an enzyme
involved in smooth-muscle relaxation.

Unlike other treatments for erectile dysfunction, the drug does
not directly cause erections, but instead enhances erectile
activity associated with sexual stimulation and excitation, Dr.
Gingell said.

The study involved 351 men with sexual dysfunction of no known
cause. The men were either given a placebo or one of three daily
doses of sildenafil: 10 mg, 25 mg or 50 mg for 28 days.

After the treatment period, 38% of placebo patients reported
improvement in their erections--while 89% of the men taking the
highest dose of sildenafil reported improved erectile activity
and sexual performance.

"The results appear to be good in men with psychogenic erectile
dysfunction," said Arnold Melman, M.D., chairman of urology at
Albert Einstein/Montefiore Medical Center in New York. "Whether
the drug works in men who have organic erectile dysfunction is
another matter entirely."

available, every man would want to try it, Dr. Melman noted, as

The principle in all these "treatments" is the same, a system to deliver a
drug to the Corpora Cavernosum tissue in the male organ to produce an
erection. The difference lies in the how to go about it. Injectables are very
effective but a non invasive system with the same results is preffered. Thus
MUSE, Gel and Sildenafil.

Muse is a semi non invasive system in which a small pellet is placed in the
urethra with an applicator, this pellet dissolves and permeates through the
skin to produce the same resullts as the injectables. It is certainly
preferable to a needle !!!!

If I had some money I would invest heavily in any of these companies.
Impotence has been a scourge to men, and women I might add, since ALWAYS !!!!
Entire herds of rhinos, tigers and bears have been wiped out because men
thought and still think, erroneously that their horns, or other organs are
aphrodisiacs.

Men are very touchy about this issue and I am very glad to see that the women
members and sysops on this forum have been the ones to bring it out. I
applaud you !!!!!!

In section 22 the "male room" we have been discussing these topics for ages.
And even though it is a "private" section and anonymity is guaranteed
participation is very small. Although we have many "success" stories and I
have a passel of private messages attesting to the help received .

I would be glad to privately or publically help any member, male or female
who needs information. I have tons of material some of which is in the
librariesand can be accessed by doing a library search with the keyword
Impotence.

At this moment there is no need to for man or woman, except in unusual
circumstances where the plumbing doesn't work to suffer the deprivation that
impotence brings.

Sex is the great leveler in life, available to all. We should not be deprived
of it.!!!!!

Just kidding Robin !!! BTW since somebody said I am running things >g> you
are elected to keep this thread going and eventually to bundle it into a very
valuable addition to our library. I am very glad to see this issue in a
"public" area (hey who cares anyway?). This is a most important topic for
diabetics (there I go again !!!) for PWD . And bringing it to your section
gives it much more exposure than in 22 or 23 . I would like to congratulate
Paula especially for bringing it up and all the women who have responded!!!

This is one of my pet topics for obvious reasons and I was getting a little
frustrated dealing with this most serious complication of DM in the confines
of the "male room" . We have been taught that sexuality is not to be
discussed in "mixed" company but hey this is almost the second millenium. And
a good and "decent" sex life is something to treasure.

<New Treatment for Impotence

Air date: November 9, 1995

Studies have shown that up to 30 million men in this country suffer from
impotence. That's 52 percent of all men between the ages of 40 and 70,
suffering
some degree of not being able to get an erection. Earlier this week, we told

about the stick-on testosterone patch, which can help improve the sex lives
of men
with low hormone levels. Now, a Menlo Park company, Vivus, has developed a
radical new way for men to deliver an erection-causing drug directly into
their own
penis, without pain.

Dr. Neil Gesundheit/Vivus Corp.: "OUR SYSTEM IS VERY SIMPLE.
INVOLVES INSERTING A VERY SMALL APPLICATOR INTO THE DISTAL
OR FAR PART OF THE PENIS, JUST INTO THE OPENING, AND LEAVING
A VERY SMALL PELLET OF MEDICATION THERE THAT DISSOLVES, IS
ABSORBED LOCALLY, AND ACTS AS A VASAL DILATOR AND BEGINS
THE ERECTION PROCESS."

In the past, a similar drug would have had to have been injected into the
penis with
a needle, a painful and unpleasant process. The Vivus drug pellet, on the
other
hand, is 5 to 10 times smaller than an eyedrop. The company says it's system
restores erections in up to 65 percent of men suffering from impotence.

OH IT'S IN YOUR HEAD, BECAUSE WE HAVEN'T UNDERSTOOD IT. BUT

The Vivus system is waiting for FDA approval. The company hopes to have it
