About a year or so ago, we had a young girl enter our emergency room complaining of abdominal pain. The nurse taking care of the patient was alredy starting our usual abdominal pain workup, including collecting urine, etc.... The patient was about 16 years old. The nurse did her assessment and took the chart to the nurse's station to add it to the growing pile of charts marked "to be seen". When the doctor finally got around to that patient, he began his normal assessment, asking the patient questions. Suspecting that she might be pregnant, he asked the patient "Are you sexually active?" to which the patient replied "No, I usually just lie there."
