Changes you must make

• Replace YOUR EMAIL with your email address.
   

• Replace YOUR NAME with your name.

• After the = after siteid, type the SiteID you were given.

   sample: &action=list&siteid=123">

How to put it on your page

If you have a non-HTML editor for your site, like PageBuilder, then choose an option to add HTML and paste the code above in. For PageBuilder specifically, click Add-Ons, choose Forms & Scripts, and then HTML code.

For HTML editors, just paste it in at the bottom of your page.
