Character's Name:The name your character uses in real life.
Name Meaning: The names in Sailor Moon all have meanings that relate to their character. Tsukino Usagi: Bunny of the Moon, Mizuno Ami: Friend of Water, and so on and so forth. Try to choose a name that has something to do with your character. Babynamer.com is a good place to find names.
Gender:Well, if you don't know what gender means that is just sad. You go now, buh-bye.

Birthdate:Birthdate as in what day they were born. Your character's birthdate determines their zodiac sign, which is an indicator of their personality.
Zodiac sign: Zodiac sign as in Taurus, Aries, Libra...
Age:How old is your character? Do they even have a determined age? Are they like forty but look twelve? Things like that.

Appearance:What do they look like? Are they prim and proper? Tom-boyish? What type of clothes do they usually where? What type of first impression do they give?
Height: How tall are they in feet and inches.
Build: Are the tiny and fragile? Slender? Curvaceous? Blah blah blah...
Eye Color:The color of their eyes and any other special features about them. Do they change color with differing moods? Do they have long lucious eyelashes?
Hair Color/Style:The color of their hair and how do they usually wear it.
Blood Type:In Japan your blood type can be an indicator of your personality.

Strengths:Are they loyal? Friendly? Intelligent? Things like that. Also do the have any *special* strengths like telepathy or something...
Weaknesses:Every good character has to have flaws, that's what makes them interesting! Perhaps they are a snob, or have bad manners. Maybe they have a medical condition of some sort.
Hobbies:How does your character like to spend their time? Reading? Biking? Playing soccer?
Family: What is your character's family like? Do they even have a family?

Transformation Phrase: [Senshi Name] [Describing word] Evolution! Example, "Gaea Orb Evolution!".
Transformation Device:What device allows them to transform? A locket? A necklace? A flower? A coin?
Transformation Sequence:What does your character's transformation look like? What is their aura color (the color their bodies glow when transforming). What pose do they finish in? Intro Speech: How does your character introduce herself/himself to their enemies? Ya gotta love a catchy intro speech. Hmm, perhaps that is why the Moonlight Knight sucked so much...

Fuku:This is what the senshi fuku looks like. You have to choose primary and secondary colors. Also describe your character's gloves, boots, earrings, hair accessories and any other alterations that need to be mentioned. Characters that aren't senshi don't have to stick to a uniform. They can wear whatever they want.

Weapon (if any): What extra things does your character use when they attack? If any thing at all.
Primary Attack: The name of the attack, what it looks like, what it does to the enemy, and the affects it has on the attacker. Does the attack have any flaws?
Secondary attack:Ditto.

History:Where did your character come from? What's their story? What has happened to them so far?
