INSTRUCTIONS

  1. Personal information: We know and understand how people feel about giving out personal information.  We require at least a name and date on the survey.  Any personal information given will be kept confidential and not released to any other party.  

  2. Completing the survey:  To complete the survey simple print it off the screen or copy it into a word processing software, fill it out and mail to:                                                                                                               WWSurvey c/o Rick Schoen

  3. Answering questions: The majority of the questions are check off Yes/No.  A few are fill in the blanks with days, percentage etc.    In the section for acute and chronic injuries, there is a box, Details Of Injury here we would like a few details of your injury such as:                    
            A. Diagnosis (type of injury- fracture, dislocations, carpal tunnel, laceration, etc.)
            B.  Date(s) of injury.
            C.  Number of episodes (example recurrent shoulder dislocations)

  4. Definitions: In the sections relating to acute and chronic/overuse injuries.  Include only those injuries that were the direct result of paddling!!!

       
          A. Acute Injury:  An injury resulting suddenly from a particular accident or episode.  Examples are, shoulder dislocations, fractures, lacerations, contusions, concussions, sprains, etc.

              B.  Chronic/Overuse Injury:  An injury or problem unrelated to a one time event.  They arise over time from repeated motions, or stresses on the body.  They are noted gradually and often reoccur.   Examples are, tendonitis, bursitis, low back pain or wrist pain (carpal tunnel syndrome).
