1. What strengths does this student/intern demonstrate as a human services professional in your agency or organization?

Jean continues to have a good working relationship with staff and youth on probation. She has a good knowledge of agency structure, function, and policies.

2. What recommendations do you have for this student/intern to enhance his/her learning and professional development in your agency?

Jean has taken the initiative to work a shift in detention, learning about the Intake process and taking the training for the diversion program for additional experience with the Juvenile Dept. She is comfortable enough to ask for assistance and advice when needed and will learn more the longer she is with the department.

Jean has demonstrated her ability to be successful in her stated learning objectives and has also developed sensitivity and insight in the cases she has observed and/or monitored.

4. What new areas of learning do you suggest for this student/intern?

Jean can expand her knowledge of the Department by working in the non-offender unit, assisting a Court Service Officer with their dependency caseload. Jean has a good understanding of how the entire Department operates.

5. What additional comments, insights, and/or information would you like to share
with this student/intern?

It is my opinion that Jean has continued to exceed her set goals and expectations of her internship. She has proven to be organized, motivated to learn, able to work independently, maintain confidentiality, aware of cultural differences, and has been a positive role model. She has a genuine interest and concern for youth in our community and presents herself in a professional manner to clients.

Rating Scale: 5 – Excellent 4 – Good 3 - Fair 1 - Poor

1. Accomplishes tasks with a minimum of supervision.

Jean has proven to be a reliable intern, one who requires little or no supervision to complete her assignments. She is very competent and works well independently.

  • 2. Is confident in internship role.

    Rating _5___

    Please explain student’s rating.

Jean seems to be gaining confidence in her ability as an intern. During the summer quarter she seemed considerably more self-assured than she had been in the past.

    3. Is well organized and efficient in accomplishing assigned duties.

    Rating_4___

  • Please explain student’s rating.

Jean accomplishes her work in a timely and efficient manner.

    4. Demonstrates problem solving by obtaining necessary information and asking for help when
    needed.

    Rating_5___

    Please explain student’s rating.

This continues to be one of Jean’s strongest points. Her confidence has grown markedly during this quarter.

  • 5. Demonstrates initiative in areas of program other than what is assigned.

    Rating_5___

    Please explain student’s rating.

This is the area where Jean has improved the most. Part of this may be the result of increased confidence, but I think her interest in the principal learning objective (behavioral changes in patients undergoing cancer therapies) is probably the biggest reason for her improvement in this area.

    6. Is professional in relationships with clients and staff.

    Rating_5___

    Please explain student’s rating.

Jean is very professional with both clients and staff. Her conduct when dealing with the staff is courteous. Her interactions with the patients demonstrate patience and compassion without losing sight of the objective.

    7. Practices confidentiality.

    Rating_5___

    Please explain student’s rating.

Jean has never violated a confidential relationship with any resident.

    8. Shows knowledge of agency structure, function, policy, and procedures.

    10. Accepts constructive criticism and positive feedback concerning performance.

    Rating_4___

    Please explain student’s rating.

Jean accepts criticism of her performance with an open mind, but I suspect she feels mildly threatened whenever this occurs. In fairness, I should add that there was very little in her work or her behavior that need any criticism.

    11. Shows sensitivity to clients and staff while maintaining appropriate boundaries.

Sensitive and understanding of the resident’s needs, cheerful and courteous towards the staff. Her behavior is always appropriate.

    Rating_5___. Please explain student’s rating.

Jean is fully aware of ethical standards at the Veterans Home and she meets or exceeds these standards in every case.

    Rating_5___ Please explain student’s rating.

Jean really took the initiative this summer, and she seemed very interested in the learning objective. We regularly discussed the status of the residents undergoing treatment, both on a personal and a clinical level. She did a considerable amount of research outside of the facility to achieve her learning objectives.
