All fireworks illegal for use in the state of Wisconsin.

Under Penalty of Law:

· I hearby understand as a condition of this sale that I have and will comply with the laws of the State of destination of the merchandise Marty’s Fireworks; and agree that I have or will obtain any necessary permits required by the law. I also agree to use or sell said merchandise in strict compliance with all applicable laws (city, municipality, county, state, or federal). I understand this is a wholesale sale and that any tax collected from out of state customers not having their state tax permit present at the time of sale will be refunded by Marty Fireworks co., when a copy of the tax permit is received at its office. I am not a local, state, or Federal official attempting to make an undercover purchase. I understand that Marty’s Fireworks, LLC, will not assume liability in any claim or injury occurring during transportation, storage, handling, use or sale of these fireworks. I understand that fireworks are dangerous and I assume all risk regarding them. I understand that I must be 18 years of age or older and have presented proper proof of my identification. I understand this sale is wholesale.

Certification

· This is to certify that the materials are properly classified, described,packaged,marked and labled, and are in proper condition for transportation according to the applicable regulations of the Department of Transportation, Title-49, part 100-199, Code of Federal Regulations and (ship) according to Trans 328, Wis Admin Code.

All fireworks are very dangerous and if not handled properly can cause serious injury. Please follow all directions on the label and packages. MARTY’S FIREWORKS will not be held responsible for any injuries or accidents due to the use of their fireworks.
