general welfare of persons who have learning disabilities.

Important...

If there have been any tests administered to your child which will enable us to better educationally prescribe for him, it is imperative that we have these results. The information which might be of use to your child's educational prescription include:

psychological data,

achievement test scores,

report card grades,

physician's evaluations,

eye and/or hearing evaluations,

anecdotal notes.

As the parent of this student, you have a right to ask for, and to receive the above information from your child’s cumulative and/or psychological files. Full cooperation from the parent, school, teacher, and ACLD will gain the best results for your child.

BE SPECIFIC IN YOUR EVALUATIONS ON ANOTHER SHEET OF PAPER, YOU MAY FREELY EXPRESS YOUR DESIRES AND YOUR CHILD'S NEEDS.

There is a reasonable fee per teaching session.
