I talked with Ernst& Young and they said that what they would like to see is the following: 1) Suggest calling Ernst & Young if you haven't received your Record of Employment and asking what you are entitled to. They will give you your vacation amount and your termination (or severance) amount. If you have received your Record of Employment, these numbers should already be on the form. 2) On the Proof of claim, 2a) fill out the top part (name, address, etc.) 2b) Total your vacation and termination amount and put in the spot in paragraph 3. 2c) Check off 4A Unsecured Claim. 2d) Check off - "I do not claim a right to a priority" IF you are expecting termination pay and write beside this sentence the following - Termination Pay = $xxx 2e) Check off - "I claim a right to a priority under section 136..." IF you are claiming for vacation pay and write beside this sentence the following - Vacation Pay = $xxx 2f) Sign and date and have witnessed the back side of the Proof of claim. 3) An attached schedule like the one that you have already put on the Sedonanotworks website needs to be attached. Ernst & Young suggested that a statement be made such as "as per information provided by Sedona Networks." I put this statement right after the severance pay claim. This lets Ernst & Young know that you are basing your numbers on what Sedona already believe to be correct. Note: It appears that the termination pay comes directly from Ontario law. I looked up the amount on the Government of Ontario webpages and found that the numbers match. See http://www.gov.on.ca/LAB/es/terminae.htm . For example, if you have been working for the company for more than a year and less than 3 years, then you are elgible to 2 weeks of salary.
