Health Promotion Program
I started this health promotion program following a high incidence of reported cases of Diabetes patients with deteriorating vision. The patients were having difficulity prefilling their insulin syringes with the correct prescribed dose of insulin which leads to high incidience of medication errors. The second problem I identified was an increase in hospitalization relating to uncontrolled blood glucose. I started a performance improvement project in order to improve our practice.
Performance Improvement Methodology
FOCUS [50] Home Care patients were identified with deterioriating vision.
PLAN 90% of identified patients reported no visit with an opthalmologist. 10% have been diagnosed with diabetic retinopathy.
DO arrangements made for all patients to have their vision checked.
CHECK Following their checkup, all patients have been diagnosed with diabetic retinopathy.

Cost Analysis of Health Promotion Program:
It is imperative that all diabetic patients from the home care program adopt this health promotion program.  This program will improve our clinical practice by decreasing the incidence of medication errors and  help reduce the frequency of hospitalization.  This program will improve patient quality of care and thereby reduce cost to the home care agency.
