Home Telehealth Program Description Home Telehealth involves the use of video conferencing or remote monitoring equipment in patient homes. In many cases, home telehealth is used to supplement traditional home nursing. The Home Telemedicine Unit supports two-way video/voice conferencing and has an electronic device port for a home blood glucose monitor, a home blood pressure cuff whereby patients can upload self-monitoring data to a central database, and a web-browser to permit patients to view their own home monitoring data, send secure e-mail to case managers, and access tailored education material at a dedicated web site links to other diabetes management resources. |
