WAIVER AND RELEASE OF LIABILITY

 

NOTE:  THIS FORM MUST BE READ AND SIGNED BEFORE THE PARTICIPANT IS PERMITTED TO TAKE PART IN EVENT SESSIONS.
BY SIGNING THIS AGREEMENT, THE PARTICIPANT AFFIRMS HAVING READ IT.

IN CONSIDERATION of my involvement at the Mid-Atlantic Classic Badminton Tournament under the auspices of USA Badminton, I acknowledge, appreciate and agree that:

1.        I risk bodily injury, including paralysis, dismemberment, disability and death, and while particular rules of the sport, equipment, and discipline may reduce this risk,
this risk of injury does exist, as well as the risk of damage to or loss of property.

2.        I knowingly and freely assume all such risk; both known and unknown, even if arising from the negligence of the releasees or others;

I willingly agree to comply with the stated and customary terms and conditions for participation.  If, however, I observe any unusual or unnecessary hazard during
my presence or participation or if I observe any concern in my readiness for participation, I will immediately bring such to the attention of the nearest official and
refrain from participation; and,

3.        I, for myself, and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, hold harmless and promise not to sue USA Badminton
and the Mid-Atlantic Tournament committee, their sponsors, their officers, volunteers, staff, sponsors and/or agents, (“releasees”) with respect to any and all injury
and loss arising from my participation, whether caused by the negligence of the releasees, the condition of the premises or otherwise,
except that which is the result of gross negligence or wanton misconduct, to the fullest extent permitted by law.

4.        I agree to be bound by the rules and regulations of the International Badminton Federation and those of USA Badminton and I hereby stipulate that I am eligible to play
in the events for which I am applying and that I understand that the above mentioned make no representation or warranty with respect to the condition of the premises
or the operation of the event.

5.        I hereby grant to USA Badminton, its licensees and contractees including photographers, television and motion picture companies, their affiliates and subsidiaries, full
television and motion picture rights including authority to film or videotape me during matches, narratives, personal interviews, or comment thereon for any and all
commercial, news or other purposes together with the right to transfer or grant their rights to others, all without remuneration or compensation to me whatsoever.

I have read this Release of Liability and Waiver Agreement, fully understand its terms, understand that I have given up substantial rights by signing it,
and sign it freely and voluntarily without any inducement. And I further acknowledge by these presents that I am aware that DRUG TESTING may occur
at this event.

“B” Men’s Singles will be held in the main facility.

 

Events:

MS, WS, MD, WD, MXD (played in A & B/C divisions) and Senior MD, with consolation rounds in all events. Entries in events will
be restricted to a nominal 16 teams per event & division.

 

Prizes:

“A” Division prize purse is $2,000 provided by USA Badminton.

 

Eligibility: (NOTE New Rules)

Open to players who are members of USA Badminton (or their National Association for non USA resident entrants). Membership
must be current at the time of playing. Annual Tournament Only memberships ($15.00) may be purchased as part of the entry. The $5
one-time fee is no longer available per USA Badminton.

 

Registration & Fees:

The entry fee is $18.00 for the first event and $15.00 per event thereafter, per person. Paid entries will take precedence over unpaid
entries should an event be oversubscribed. Entrants will be accepted on a first come basis based upon postmark date. To qualify,
entries from BOTH partners in doubles events are required, the later partners entry date will determine the entry date. At the tournament
director’s discretion late entries may be accepted if an event is still open after the deadline. Any such late entries will be subject to a $10
per event per person surcharge. Events may be closed prior to the deadline if a large number of entries for an event are received.

Yonex feather shuttles will be provided.

Friday, February 23                              Registration / Warm-up 4:00pm; WS 5:00pm; MS 7:00pm

 

Saturday, February 24                         Registration / Warm-up 8:00am; “B/C” MD & WD 8:30am;  “A” MD & WD 10:00am

                                                                MXD & Snr. MD 4:00pm. Tournament party TBD. (see website for details).

 

Sunday, February 25                           A Finals, 10.00am (Order to be determined by finalists).

Regrettably there will be no courtesy transportation at this years Mid Atlantic Classic.

Directions:

FROM PA Turnpike:

From West Exit 24 (Valley Forge Interchange) and follow the signs to I-76 East (Schuylkill Expressway to Philadelphia).
Exit I-76 for I-476 South. Proceed to Exit 2 (Swarthmore)
of Rte I-476, turn left at first traffic light.

From East Exit 25a (Mid-County) Rte I-476. Proceed to Exit 2 (Swarthmore) of Rte I-476, turn left at first traffic light.

 

FROM ALL POINTS SOUTH: Take I-95 North. Approaching Chester PA, take exit 7 onto I-476 N toward Plymouth Meeting.
Take Exit 2 (Swarthmore). Turn right at first traffic light.

 

All proceed East to junction Rte 320 (Sproul Road). Turn Left. (Springfield Mall is directly to your left). At 3rd light on 320
excluding the light you just turned at turn left into the Springfield Hospital Complex. Stay to your right and go all the way
to the multi-story parking lot at the end of the road. Park. Entrance to the Hospital and Healthplex are to the left as you first
enter the parking lot.
