Paperwork and Supplies

1. Emergency, medical and DCFS forms are to be filled out prior to child’s first day. All forms will be updated yearly or on an as needed basis.

2. If at any time you cannot be reached at the work phone number I have on file I must be notified and supplied with a number at which a parent can be reached. Parents need to be reachable at all times, preferably in one call. No child will be released to anyone other than those listed on the Child Pickup Form.>

3. Provider will provide:

Parent will provide:
_____formula
_____diapers
_____wipes
_____minimum of 2 changes of clothing
_____outdoor clothing
_____infant carseat

Two complete changes of play clothing must be left for each child. Children will get messy. During potty training child will require an additional change of clothing and plastic training pants. Clothing will be exchanged seasonally and as the child changes sizes. Seasonal clothing includes shorts, tee shirts, bathing suits and sunhats for summer and coats, hats, mittens and boots as needed for winter. Closed toe shoes are preferred. If sandals are worn to child care please bring socks and closed toe shoes with you.Sunscreen is to be applied by parent before arrival. Provider will reapply as needed throughout the day.

4. I require a 3 week trial period. Care may be terminated within the trial period by provider or parent without notice. After the trial period either parent or provider may terminate childcare by giving a 3 week minimum written notice. Payment is required for the full 3 weeks even if services are not used. If the provider feels any child is a danger to the other children care may be terminated immediately.


Daily Schedule

5. Arrival and pickup times are individually contracted and must be adhered to unless prior arrangements have been made. All arrivals will be prior to 8:30am. Please call if your child will be arriving later than 8:30am. Please try to schedule doctor and other appointments for your child near the end of the day.

6. The State of Illinois requires that authorities must be contacted should a child be left at a childcare facility after business hours. The provider will attempt to reach a parent by phone and if unsuccessful call the people listed on the child pick-up list. If no contact and arrangements have been made by 6:00pm the police and DCFS will be notified and the child will be released to those authorities.

7. I serve nutritious meals to the children. I belong to the Illinois Children’s Alliance Food Program. Breakfast is served at 8:15 am, lunch is at approximately 12:15pm, snack is at 3:30 pm. If your child arrives after 8:30am please feed them breakfast at home. Please do not bring food to eat here after breakfast is over. Babies are fed on demand, unless otherwise indicated.

8. My field trip policy is: We take occasional field trips. I will post field trips either in a newsletter or on the bulletin board in a timely manner. I will pay all admission fees. Occaisional contributions toward field trips and classes may be required. Field trips may be cancelled or postponed at the provider’s discretion. If you do not wish your child to attend the field trip you are responsible for alternate care for the day. I will be paid as usual since I am available for care for the day.

9. My policy on toys is: with the exception of “blankies” or other comfort toys needed for naptime please have your child leave their own toys at home. If you bring toys in the car, please leave them in the car. I will provide all toys as needed. I have found it is too difficult for the children to share toys from home even if they share nicely under other conditions.


Holidays and Vacations

10. My holiday policy is: I require the following holidays off with pay –New Year’s Day, Memorial Day, Independence Day, Labor Day, Rosh Hashana, Yom Kippur, Thanksgiving Day and Friday after, Christmas Day. With the exception of Rosh Hashana and Yom Kippur if the holiday falls on Saturday I will be closed the preceding Friday. If the holiday falls on Sunday I will be closed the following Monday.

11. My vacation policy is: I will be taking 3 weeks of unpaid vacation yearly. I will notify you as far in advance as possible. Notice will be given in announced dates attached to this agreement, in the newsletter, and posted on the bulletin board. If parents take vacation at other than the providers planned vacation time, full payment for your child’s slot is required. Fees will be due on the last day of attendance prior to the vacation and may be held until Monday if requested. Please notify me as far in advance of any vacation as possible so I may plan activities accordingly. Aid in finding backup care for vacations will be provided if requested.

12. I will be taking up to 5 paid days yearly for training (conference) purposes. I will be taking up to 3 personal paid days yearly.


Illness and Discipline

13. My policy on illness is: contagious illnesses, diarrhea, vomiting, fever over 100 degrees F, and undiagnosed rashes:child needs to be picked up as within 1 hour and remain at home until symptom free for 24 hours. While waiting for you your child will be kept comfortable in a room away from the other children. If your child is vomiting or if your child’s temperature requires use of medication to keep it down please realize your child is still ill. Your child needs to be able to maintain normal temperature without the use of medication to be well enough to return to my home. Some illnesses may require a doctor’s note to return to childcare. If a child is too ill or irritable to participate in normal activities please keep your child at home until they are ready to participate in normal activities. Please notify me if your child has been exposed to any contagious illnesses. If your child has shown any symptoms in the prior 24 hours please tell me. This may or may not be cause to remain at home. But I can provide better care by knowing what to look for. No adjustment to fees will be made for sick days.

14. My policy on provider illness: If I am unable to provide care due to illness I will notify you as early as possible. Parent is responsible for back up care. Should an emergency occur that forces closure for more than 2 days fees will be prorated.

15. My policy on discipline is distract and redirect. If needed time outs will be used.


Changes to Agreement
16. Provider retains the right to change policies at any time within the contract term with written 3 week notice.

17. Failure on the part of the provider to enforce any portion of these policies does not constitute a change in policies nor does it negate any portion of the agreement.



House Rules

Parents will drop off and pick up at the side (kitchen) door.

No shoes past the entry way please.

Sign in/sign out book is located on the wall near the door. Children must be signed in and out daily.

Quick dropoffs and pickups are much easier on the children. I can always be reached by phone, voice mail or email for any concerns you might have. Please leave a message and I will respond promptly.

Please respect my neighbors and do not block their driveway.

For safety reasons parents must maintain control of their children when arriving and leaving child care. Please take your child by the hand if necessary.

Children should never be left unattended in the car.

At no time will children be allowed to run, jump on furniture, wrestle or climb in the house.

At no time will children hit, kick, bite, push or otherwise hurt anyone. Exceptions will be made for toddler biting, and I will do my best to teach toddlers not to bite as quickly as possible.

At no time will children throw or intentionally break anything.

At no time will children pick up babies or toddlers.

Together we can make Mrs. J’s Family Child Care a happy place for all children.


Child Care Agreement

1.The following agreement is made between __________________ and ________________________ and Nancy Jandt (provider).

2.Care for ________________________________________will be provided on ________________for the hours of ____________________.

3.This agreement will be renewed on a yearly basis in the month of June.

4.My fees are as follows:
a. A non refundable deposit equal to two weeks fees will be due either upon enrollment or paid out as agreed to. This will become your final two weeks fees upon written notice of termination of this contract.

The rate schedule at Mrs. J's Family Child Care will be structured in the following manner if this option is chosen. __________per week if paid by pickup on the Friday previous
to the week of care.
__________per week if paid by Monday morning of the week of care __________per week if paid by Monday evening of the week of care $5.00 per late day charges thereafter. If fee is not paid by Tuesday care may be refused until weekly fee and all late payments are made.

c. In the unpleasant event of a NSF check you will be liable for any bank service charges I incur as the result of the NSF check. Thereafter, payment may be required in cash.

d.There is no refund for early pickup. Late fees are $5.00 for the first quarter hour past contracted pickup time and then $1.00 per minute thereafter. This fee is to be paid before the child is dropped off the next day. Exceptions m ay be made at t he providers discretion for bad weather or when prior arrangements are made.

I understand that the provider is a mandated reporter of suspected child abuse or neglect.

Termination voids this agreement.
