Viktor E. Frankl consistently used the term spiritual free from any religious connotation wishing to keep Logotherapy and religion strictly apart. However his dimensional ontology implies the possibility of extension with an all-encompassing fourth dimension, to which religion and theology belong. This fourth, pneumatic dimension (pneuma is Greek for „spirit”, meaning „Holy Spirit”) was explicitly introduced by American logotherapist Donald F. Tweedie, Frankl’s personal friend and disciple. Moreover, the pneumatic dimension constitutes the specific field of work for a holistic approach to healing like Bernard J. Tyrrell’s Christotherapy where Christ is the Logotherapist par excellence. This latter attemps the integration of the process of religious conversion and spiritual exercises with psychotherapy and also with Logotherapy. The pneumatic dimension extends to the unconscious depths of the human soul, as it can be discovered in experiences of mystics like John of the Cross, and it reaches to the social level as well, forming a “pneumatosphere” which is our common spiritual environment.

Viktor E. Frankl hat das englische Adjektiv spiritual (geistig) stets frei von jeglicher religiösen Bedeutung verwendet, da er Logotherapie und Religion strikt voneinander getrennt zu halten wünschte. Seine Dimensionalontologie mag jedoch die Möglichkeit einer Erweiterung in Richtung einer vierten Dimension zu geben zu welcher Religion und Theologie zählen. Diese vierte, pneumatische Dimension (von griech. pneuma, Geist; gemeint ist jetzt der Heilige Geist) wurde ausdrücklich vom amerikanischen Logotherapeuten Donald F. Tweedie, ein persönlicher Freund und Schüler Frankls, eingeführt. Darüber hinaus konstituiert die pneumatische Dimension den spezifischen Aufgabenbereich eines ganzheitlichen Ansatzes wie etwa Bernard J. Tyrrells Christotherapie, die in Christus den Logotherapeuten schlechthin erkennt. Dieser letztere Ansatz unternimmt eine Integration von religiöser Bekehrung und geistlichen Übungen mit Psychotherapie und ebenso mit Logotherapie. Wie die Erfahrungen der Mystiker, etwa eines Johannes vom Kreuz, zeigen, reicht die pneumatische Dimension bis in die unbewussten Tiefen der menschlichen Seele hinab und erreicht ebenso die soziale Wirklichkeit, indem sie als unsere gemeinsame spirituelle Umwelt eine „Pneumatosphäre“ formt.
