This packet contains a summary of the consultation, a brochure highlighting the procedure you are interested in, and a specific quote for the cost of surgery detailing all fees. |

determining what treatment alternatives are available, he will explain to you
the benefits and expected results of the treatment as well as the risks and
complications of each alternative. You will have a good understanding of the
required aftercare and expected recovery.
