Polonius is not shy about telling Claudius what he should do. He also feels at liberty to be fairly meddlesome particularly with regard to Hamlet. He decides that Ophelia will not be permitted to see Hamlet and he apparently does so without any input from Claudius or Gertrude. 

When Polonius finds out what he believes to be the source of Hamlet's problem he brings it to the attention of the King but he delays the delivery of that news until they hear from the ambassadors first. The King and Polonius clearly have their own theories about Hamlet's transformation and Polonius is insistent in pursuing his own theory. This it turns out is his own undoing. Claudius wants to send Hamlet away after the nunnery scene but Polonius wants a chance to explore his theory and contrives a means to have his mother confront him. Claudius relents and Polonius is slain.
