Grade Sheet Adjustments

 

 

PROCEDURE:

 

1.   Have the changes assessed by the Records Officer;

 

2.   Pay the amount due to the Accounting Office;

 

3.   Present the receipt to the Registrar’s Office for issuance of Grade Sheet Adjustment Slip;

 

4.   Photocopy the Grade Sheet Adjustment Slip;

 

5.   Furnish each professor a copy and have them acknowledge the changes in the original GSAS;

 

6.   Secure a copy of the acknowledged GSAS and have it received by the Registrar’s Office
