Leave of Absence

 

 
  • Students who do not wish to enroll in the succeeding semester or who wish to cancel their enrolment in all subjects during the first two weeks of classes must drop all subjects and apply for a leave of absence (LOA).

 

  • LOA is officially granted for any of the following reasons:  1) Death of an immediate

member of the family 2) Poor health 3)  Other reasons approved by the Dept. Head.

 

  • A student who takes an unofficial or disapproved leave is placed on AWOL status.  The school has the right to refuse automatic admission to a student upon his return.

 

  • Each Leave of Absence contract must not be more than two (2) consecutive semesters.  However, the LOA contract may be renewed before the end of the previous semester

 

PROCEDURE:

 

1.  Present your OSL-Grade Report to the Registrar’s Office and have your grades Evaluated and validated;

 

2.  If qualified, submit letter of intent signed by the parent or guardian to the Registrar’s Office

 

3.  Accomplish dropping form and have all professors sign for subjects currently enrolled in;

 

4.  Accomplish Application for Leave of Absence form;

 

5.  Get endorsement from the Guidance Counselor;

 

6.  Secure Approval from the Department Head;

 

7.  Validation, official stamping, and recording by the Registrar’s Office
