The founding of the SHS was a collaboration of many individuals and institutions. Foremost among them was the late Dr. Florentino B. Herrera, Jr., who initiated the founding of the school while he was Dean of the U.P. College of Medicine. The names listed below were members of the Committee of the Institute of Health Sciences. They affixed their names and signatures on the document, "Proposal for the Establishment of the U.P. Institute of Health Sciences Tacloban." The proposal was submitted on April 14, 1976. It was approved by the U.P. Board of Regents on April 19, 1976.
