Physical

Aside from using the altered action resolution system, very little has changed.

Damage Effects

Knockout and Bleeding

Unconscious

Death and Permanent Injury

If your health is reduced to a negative value greater than your Death rating, there is a chance you might die.  You will need to make a Body check against a difficulty based on how much damage you have taken, failure means death.  If you succeed, you suffer a permanent injury.

Mental

In the original BESM2 system mental damage is taken off of Energy.  I didn't like the ideas of superheroes that use their powers becoming more susceptible to mental domination.  So the new derived stat Psyche was made to handle mental damage.

Temporarily Insane

sometimes this can be a form of terror, sadness, or just being stunned

Suggestible and Unmotivated

Catatonia and Permanent Insanity

If your Psyche is reduced to a negative value greater than your Insanity rating, there is a chance you might become completely insane and non-functional.  You will need to make a Soul check against a difficulty based on how much mental damage you have taken, failure means you are catatonic.  If you succeed, you suffer some permanent insanity or quirk.
