Current Medical Error theory supports the sharing of "stories" of Medical Errors in order to raise awareness among health care providers.  We would like to assemble a compendium of such stories in print and feature one of these on occasion on the website.  Stories may be submitted here for possible inclusion.  We reserve the right to omit and/or edit any submissions.  All submissions will be edited to remove unique identifiers and preserve confidentiality.
