Compared to the anime, the manga story of Hotaru is a little bit different. At the age of eight she was badly hurt by a fire in her father's laboratory and her mother died there. She might have died too, but her father, Professor Tomoe, a scientist who tried to form a new kind of human being, a super human, tries to keep her alive. He thought the best way would be to do so by replacing certain parts of her body by mechanical devices. It is not sure if he acted out of love or if he just wanted to use his knowledge making her his favorite experiment. He succeeds and Hotaru is kept alive, she grows up but she is a very sick girl, she faints often and she suffers from terrible headaches. She senses that there is something evil deep inside her that wants to break free. From time to time this evil part of her takes over and then she shows aggressive powers and her eyes are empty. The other kids in her class are afraid of her and although she is the daughter of Professor Tomoe, who owns the school she attends, the other kids don't want to come too close to her. Her father seems to be very worried and he gives her an amulet, a crystal seed on a chain, but it doesn't help much neither does the medicine her father gives her. From time to time her father tries to improve her body and she is shown with lots of wires coming out of her body. It is not clear if she is just an improved human being or something completely new, Uranus calls her a combination of human and alien and says that her body is made from different separate parts (like Frankenstein's monster?).
The other Senshi of the Outer Solar System attend the same school but they never have a clue that inside Hotaru there is Mistress 9, the main leading Daimon, waiting for her chance to awaken, but they are aware of the fact that Hotaru is the reborn Sailorsaturn. They think that the struggle inside Hotaru is a sign that Sailorsaturn is about to awaken and her appearance is going to destroy the world again. It happened a long time ago but Uranus, Pluto and Neptune still remember that after they had heard the death cries from the Moon Kingdom, the light of their talismans summoned Saturn. The Senshi of Rebirth raises her glaive and everything is destroyed to be reborn again. and they don't want her to awaken, that's why they want to attack her but Chibimoon tries to save her.
Just like in the anime, the evil inside Hotaru is attracted by the ChibiUsa's brooch. Hotaru has managed to withstand this need all the times the two girls have been together before but this time she snatches Chibimoon's brooch. Chibimoon re-transforms into ChibiUsa and her body is deadly cold because her Ginzuishou inside the brooch is linked to her life force. Hotaru's hair starts to grow incredibly fast, her body grows too and a black star appears on her forehead - Mistress 9 has awaken, Hotaru is gone.... or isn't she? Inside Mistress 9's body there is Hotaru's soul, still fighting evil, still trying to right the wrong that has been done to ChibiUsa. Without the Ginzuishou, the little Senshi has lost her soul too and its trapped inside Mistress 9 just like Hotaru's. Later Hotaru has to protect the four Inner Senshi's souls too (Mistress 9 has swallowed them as she has swallowed the Ginzuishou before). Although she is trapped inside Mistress 9 Hotaru knows that her father has died and she also knows that she is dead herself. Mistress 9 (Professor Tomoe has planted her egg inside Hotaru during his experiments), wants to destroy her human "shell" but Hotaru doesn't let her go. She feels that there is another, stronger "self" hidden somewhere and this stronger self can withstand Mistress 9's powers. Finally it is Hotaru herself who breaks free and carries the souls of the four Inner Senshi back to their bodies and ChibiUsa's Ginzuishou back to her friend. After she has given the Ginzuishou back to ChibiUsa, she says good-bye, the evil creature Mistress 9 destroys Hotaru's altered body and Hotaru's soul vanishes. Mistress 9 is killed by Pharaoh 90 and the three Senshi's of the Outer Solar System talismans start to glow. Sailorsaturn appears out of Mistress 9's head and she is ready to destroy the world in order to destroy Pharaoh 90 as well. The others beg her to stop but she does, what has to be done, she shouts "Death Reborn Revolution", thousands of ribbons come out of her glaive. Pharaoh 90 wants to consume the light of her attack as it has consumed the energy of the other senshi's attacks before but this time the power is too much to be swallowed ... Saturn commands it to go back to where it has come from and a beam of light shoots up to the dark galaxy's center. Saturn raises her glaive and brings it down and it looks as if this is the end of it all ...The high skyscrapers break apart and Saturn watches Pharaoh 90 fleeing towards his home, she wants to take care that he is really gone, she won't destroy the world but save it instead because of the power of the Ginzuishou and the Holy Grail that was given to her by Sailormoon, she enters the gate of darkness, commanding Pluto to close it behind her.
