    Fees for members and non-members are inclusive of bank charges, conference kit and papers, entitle entry to all sessions and activities (except closing dinner), as well as provision of lunch plus morning and afternoon teas on each of the five days. (Non-members who apply to join SEAPAVAA before or during the conference pay the member's rate.
            Fees for accompanying persons provide the same entitlements except for the conference kit, papers, and entry to the symposium sessions.
PREFERRED ACCOMMODATION:
    Please tick your choice. Delegates stay at the Arnoma Hotel, 99 Rajdamri Road, Pathumwan, Bangkok 10330. The hotel is located at the center of downtown Bangkok. A special rate of US$55 per room per night, including buffet breakfast, tax and service charge, has been negotiated.

CANCELLATIONS:
    In the event of cancellation, registration fees are non-refundable.
CLOSING DATE:
    The official closing date for the conference registration is 22 June 2001. Late registrations will be accepted but attract a surcharge of 10%.
