06/20/60 From records in the Volusia County Courthouse I was born as "Baby Girl Cornelison", in Ormond Beach Osteopathic Hospital, Ormond Beach, Florida. Have been told that birth mother's name was Celeste Cornelison who was a "school girl" at the time. Birth father's name is unknown, only info on birth father is that he was in a band.
