A range of community services to the frail aged, younger disabled and their carers is offered to residents in Rockingham and Kwinana and Mandurah.

These services include;

  • Respite
  • Home maintenance
  • Personal care
  • Social support
  • Adult day care.

    As these services are funded by the State and Commonwealth Governments, receiving a service is not dependant upon your ability to pay.

    Home Care Services

    The Coastal Hub Community Services is operated by Churches of Christ Homes and Community Services Incorporated and provides a range of services to the Communities of the South West Metropolitan Region. These services are designed to support and maintain people with frail aged, younger disabled or dementia that meet the HACC criteria. in their homes.
    Some services are funded by the Department of Health under the HACC Program. The services provided and where they are available are:

  • Respite Care (Kwinana, Rockingham, Mandurah)
  • Home Maintenance and Gardening (Rockingham)
  • Personal Care (Kwinana, Rockingham)
  • Social Support (Rockingham and Kwinana)

    Veterans Home Care

    Veterans' Home care (VHC) is a service funded by the Dept of Veterans Affairs to provide low-care services to veterans and their spouses.
    The aim of the program is to allow you to remain living in your own home longer.

    The Host Family Program.

    The Host Family Program is an initiative of the Commonwealth Department of Health and Aged Care through the National Respite for Carers Program, and Churches of Christ Homes and Community Services incorporated.
    Frequently, those providing permanent care for people with dementia or the frail aged need to have a break away from the demands of providing constant attention to the person in their care.
    The Host Family Program provides up to 66 hours planned or emergency respite with a host family in their home environment.
    The aims of the program; Providing respite care with staff or volunteers who are matched (linguistically or culturally) with the carer or client.
    Providing care in circumstances where the absence of such care could result in some risk to the health and well-being of the person being cared for.
    The program operates in the South West metropolitan. Upper Great Southern and South West areas of Western Australia.
    Referrals to be assessed for a Host Family occasion can be made by your regular doctor, an allied health worker, friends or family, or you can contact the office directly yourself.

    Kwinana Adult Day Centre.

    The Kwinana Adult Day Care Centre, located at 28 Christmas Avenue, Kwinana, is operated by Churches of Christ Homes and Community Services Incorporated.
    This is a Department of Health, Home and Community Care (HACC) funded service for the frail aged and dementia people of the Kwinana community.
    The Day Centre primarily provides a respite service to support those who meet the HACC criteria and care for or are frail aged or have dementia.
    The day centre provides a home-like environment where clients can meet other people, enjoy individual or group activities and take part in social outings.
    This time grants carers the opportunity to rest, pursue other activities, develop social relationships, enjoy recreational activities or attend to other matters.

    Ray Street Care Centre;

    The Ray Street Adult Day Centre, located at 20 Ray Street, Rockingham, is operated by Churches of Christ Homes and Community Services Incorporated.

    This is a Department of Health, Home and Community Care (HACC) funded service for the frail aged, younger disabled and dementia people of the Rockingham community.

    The Day Centre primarily provides a respite service to support those who meet the HACC criteria and care for or are frail aged, younger disabled or have dementia.
    The day centre provides a home-like environment where clients can meet other people, enjoy individual or group activities and take part in social outings.
    This time grants carers the opportunity to rest, pursue other activities, develop social relationships, enjoy recreational activities or attend to other matters.
