FormMail
FormMail is a generic WWW form to e-mail gateway, which will parse the results of any form and send them to the specified user. This script has many formatting and operational options, most of which can be specified through the form, meaning you don't need any programming knowledge or multiple scripts for multiple forms. This also makes FormMail the perfect system-wide solution for allowing users form-based user feedback capabilities without the risks of allowing freedom of CGI access.

BNB Form
An easy to install, muti-featured form-email script. Features include things such as autoresponding to submitter, indicate required fields to be filled in before user can submit, and more!

GMFormMail
AF² is an easy to use, Form to Email gateway. It uses simple templates to format all aspects of its output- you are in full control over everything it outputs. It can be setup to work as a multi-user script- so an ISP only has to install one copy for everyone to benefit from it. It can send out more than one email at once- so for instance, you can have it send one email to you, a different courtesy reply to the user, and another to your boss.

Alien Form v2

EZ Form Mailer
This CGI script processes the information entered into a HTML Form and reformats it into an e-mail message. This message will then be sent to the destination you specify. The information the script uses to reformat the form data and send the e-mail message can be entered directly into the script or supplied via hidden fields in the form being processed.

Form-to-Email
Form-To-EMail is a very simple Perl script that processes general HTML forms and emails them to a specified address by using hidden HTML field. Using hidden HTML fields, you can also specify the FROM, SUBJECT, and FOLLOW UP URL fields.

Form-to-File
I'm sure everyone has tried those forms that put the data in your mailbox. If you have a high traffic site like mine then you know your mail box is going to get pretty flooded. This script will store the data from a form to your web server.

Cliff's form mailer
This is a pretty simple script. It takes the data posted from a form and mails it to you. It can be set to email a response to whoever submits the data, and it validates email addresses.

Soupermail
Soupermail is a free, generic, highly configurable HTML form to email CGI program. Its written in Perl and runs on UNIX and Windows NT based webservers.

Nether mail
This easy to use script, can be used for all types of form processing.   All you have to do is modify three lines, and it will process and send any Form to your e-mail address. 

Mailto
A very simple form-to-mail script. Simple, but not as many features as some other scripts.

Advanced Form mailer
Advanced Form Mailer is a perl script for parsing web based forms. All configuration for the script is taken from the form input. This makes it highly portable and extremely useful for applications that would otherwise require multiple scripts.
