Ok heres what ya gotta do to get my awards

As you can tell I now have 3 awards.. so when ya fill out the form,Please Please Please tell me which one you are applying for.If you dont tell me what award you are applying for, I cannot even think to give you an award because, I donot know what award you would like. Even if your site is Awesome. Also.. If you sign my guestbook, Either EMAIL me or FILL OUT THE FORM. That way i know you applied. Now.. On with the rules!! *grin*
