As a patient, we would need you to provide basic information about
your general health including current medications. Unless prior
arrangements have been made, any copayments or deductables are due
at the time of service. As a courtesy, we would file insurance claims
for you. Please bring your insurance card to every appointment.
SCHEDULING: We will do our best to accommodate your busy
schedule. In order to assure adequate time for all patients, we
ask that you notify us at least 24 hours in advance of any need
to change an appointment. Multiple broken appointments may result
in dismissal from the practice.
Emergencies: In the event of a dental crisis, an emergency
contact system is in place to help you reach the on-call dentist.
We also set aside a variety of appointments times throughout the
week for emergency patients
