This is the parent evaluation form I have my clients fill out once a year. I like it because is not just checking off yes or no, the parents have to write in an answer. I supply a sealed box that the parents can drop the finished evaluation into and then check it at the end of the week. This way the parents may fill them out anonymously if they choose to.
