5th Grade Classroom Discipline Plan

Dear Parent(s) / Guardian(s),

It is with pleasure that we welcome your child to the fifth grade.  In order to provide our students with the excellent educational climate they deserve, we have developed the following Classroom Discipline Plan that will be in effect at all times.

Rules:

  1. Listen carefully.
  2. Follow directions.
  3. Raise your hand before speaking.
  4. Respect school personnel and your classmates.
  5. Work quietly.  Do not disturb those who are working.
  6. Respect school and personal property.

If a student chooses to break a rule:

1st time:            Warning

2nd time:            Fill out behavior journal, discussion with teacher, recorded on behavior record

3rd time:            Lose a privilege, parents notified, recorded on behavior record

4th time:            Lose all privileges, parents notified, principal notified, recorded on behavior record

Severe disruption or infraction: Immediately sent to principal.

Students who behave appropriately will be positively rewarded with praise and special class activities.

It is in your child’s best interest that we work together with regard to his or her education.  We will thus keep you informed of your child’s progress in the fifth grade.

We have discussed this discipline plan with your child, but would appreciate it if you would review it with him or her before signing and returning the attached form.  Please retain this letter for future reference.
