Underdrive pulleys lower the rpm of the driven accessories: the water pump, power steering pump, A/C compressor, and the alternator. Some people claim there is a significant increase in horsepower. The power steering and a/c do have a large drag but if you use you air much, the battery will not stay charged unless you average above 4000 rpm when driving.
