By signing this contract, you agree to the terms and conditions outlined in the handbook. This contract will cover care until the first of January of the year signed, unless otherwise noted. Tuition may, at my discretion, increase periodically, but not during the term of this contract. In this contract the terms "I", "We" and "My" mean the parents and the terms "Provider", "She" and "Her" refer to Phyllis Parmelee.
We (I), ______________________& __________________________, have received and read the 100 Acre Wood Childcare Handbook and will comply with all provisions contained therein, and shall at this time enter into an agreement with Phyllis Parmelee, of 100 Acre Wood Childcare for the care of my (our) child(ren)

A rate* of $ ______/week, $ _______/day, $ _______/hour, $ _______/month will be charged. This fee is to be paid on each Friday for the following week's care. These rates will be charged regardless of the child's attendance. The contracted days and hours are as follows:

Lack of enforcement of a certain policy at any time does not indicate that the particular policy is no longer in effect.
A non-refundable security deposit of $___________ is required, which will be credited towards the child's last two weeks, as fee's may change. The yearly supply fee is also due at this time and every January 1st thereafter.
If you give permission for Phyllis Parmelee to transport you child/children in her car for emergencies, fieldtrips, outings or short errands please check here __________.

Either party with proper notice may terminate this contract at any time for any reason. Proper notice will consist of written notice to the provider not less than two weeks prior to the child's last day of care. In this case, my security deposit will be credited towards the last two weeks of care and I will only owe the difference in rates from the year my children enrolled to the year care was terminated. If I choose to terminate care with less than proper notice, I agree to forfeit the entire deposit and regular charges equal to two weeks of care will be owed by me. Also, if I choose to terminate services before services actually begin, but after deposit has been paid, I forfeit my entire deposit and may be charged tuition for the weeks that an unpaid slot was held that could have been paid by another child.
If, at any time, after consultation with myself (the parent), Phyllis Parmelee feels that I or my child pose a safety risk to any of the children in her care, she will give me as much written notice as possible that care is terminated. This action will be reserved for extreme cases only, and she will attempt to resolve any issues with me before resorting to termination of care. Under these conditions, forfeiture of the deposit will be at her discretion.
