Mabe Village
Link fell down from the well above and discovered a Piece of Heart awaiting him.

Mysterious Forest
Once Link had the power bracelet, he entered the log tunnel and moved the stones to find a Piece of Heart.

Kanalet Castle
Once Link got the flippers, he swam all the way around the west side of the castle. In the southwestern screen he dove near the bottom of the screen to find another Heart Piece.

The Fishing Pond
If Link could catch all the fish in the fishing pond, the last fish he caught would have a Piece of Heart.

Ukuku Prairie
Link used a bomb to find the secret cave, then used the Pegasus Shoes to break the crystal blocks, and find a bombable wall. Link used anothe bomb to blow up the wall and find a Piece of Heart.

Tal Tal Heights Water Cave
After Link was done fishing in the Angler's Cave, he went right several screens until he found another cave. He went inside and dove underwater until he came out with a Heart PIece.

Eastern Tal Tal Mountains
Link crossed the rope bridge, and whacked down the bush. After he was finished with the bush, he could now get the Piece of Heart.

Western Tal Tal Mountains
After exiting Turtle Rock through the back exit, Link seen a Piece of Heart in plain sight and decided to go and grab it.

Koholint Prairie
Link needed the help of the Roc's Feather to jump over the pits surrounding this Heart Piece.

Yarna Desert
After defeating Lanmola, Link fell into the sand pit. Down there, he bombed the north wall to find a Piece of Heart.

Animal Village
Link took the path outside the eastern fence. Link used his hookshot to reach the Piece of Heart that was just a few feet away from him.

Cemetery
Link went to the cemetery secret passageway, and pushed up on the tombstone. Link could have used his hookshot to reach this Heart Piece of he could have just jumped diagonally to get it.
